Select Home Health, LLC is a private for-profit home health agency licensed in the state of Texas, certified by Medicare, and fully accredited with Deemed Status by Community Health Accreditation Parter (CHAP). Select Home Heath, formerly known as Fellowship Home Health, was established in 2006 and certified by Medicare in 2008 to provide exceptional care in a compassionate way to patients in the comfort of their homes.

Physical therapists provide individualized evaluation, gait training, transfer training, gross motor skills, home exercise programs, and other therapeutic treatments in the comfort and privacy of your own home. Therapists are available seven days a week.
Physical Therapists
Medical Social Workers provide information and referrals to community resources, assistance with nursing home or assisted living placement, help in applying for Medicaid or other entitlement programs as well as counseling for the patient, family, and long term planning.
Social Workers
Home Health Aides provide personal care, assist with bathing, dressing, meal preparation, and home exercise programs under the direct supervision of the professional nurse or physical therapist in conjunction with other skilled services.
HOME HEALTH AIDES
Registered Nurses personally meet with each patient, family, and/or loved ones to perform the initial assessment. This comprehensive evaluation guides us in creating a "Plan of Care" that is individual to each patient. Our staff is then oriented to the distinctions of the individual and can therefore provide services specific to their needs.
